In the last 6 months, have you started, stopped, or changed a hormone medicine, or had fertility treatment?

Has a doctor told you that you have any of these?

Do you have diabetes?

If you have diabetes, is it being managed and checked by a doctor at the moment?

Do you get keloids, or thick raised scars, when your skin is injured?

After a cut, a burn, or a spot, does your skin often leave a dark mark or a lighter patch that lasts for weeks or months?

Do you have epilepsy, seizures, or migraines that are set off by flashing or bright light?
